
Clinical Health Services
Within public health, clinical health services are concerned with preventing disease, education, and wellness. CNHD’s clinical health services include chronic disease prevention, immunization, maternal child health, tuberculosis, family planning, and STI services.

- All services offered on an income-based sliding scale fee basis.
- Confidentiality is guided by strict confidentiality laws. Your health information will never be disclosed to another person or entity without your written authorization.
- We accept cash, checks, and credit cards. Payment is due at the time of service.
- We bill Medicaid, and some commercial insurance plans.
- Appointments recommended – Walk-in appointments are limited.

- Many vaccines require eligibility under the Vaccines for Children (VFC) program or 317 program for adults.
- Limited vaccine available for those with private insurance.

- Birth Control
- Free condoms available upon request.
- Sexually Transmitted Infections
- Tuberculosis Screening & Education

- Sharps container disposal & exchange
- Community Health Worker – assistance with resources based on individual needs.
- Car Seat Program
